Clinic cancellations and no-shows: what they cost and how to cut them

Guide · ClinicsBy 7 min read

A no-show costs your clinic the price of the appointment, because you can't sell that time again. The cheapest ways to cut them are a clear cancellation policy, a reminder a day or two before, an easy way to cancel at any hour, and a short waiting list to refill late gaps. Deposits and late-cancellation fees can help too, as long as they're fair and reflect what you actually lose.

This guide shows how to put a number on missed appointments, why patients miss them, what a fair cancellation policy says, how reminders fit within the rules on marketing messages, and why answering the phone matters more than it looks.

Figures and rules checked on 31 October 2026. This is general information, not legal advice; take advice before you change your terms.

What do no-shows and late cancellations cost a clinic?

No-shows a week, multiplied by your average appointment price, multiplied by the share of those slots you don't refill, multiplied by 52. Then take off any deposits or fees you keep.

As an example only, for a clinic with an average appointment price of £60:

  1. 12 no-shows a week × £60 × 52 = £6,240 a year in empty slots.
  2. 21 late cancellation a week that you can't refill × £60 × 52 = £3,120 a year.
  3. 3Together that's £9,360 a year, before you count the patient who wanted that slot and booked elsewhere.

Your own diary will give you better inputs. Most practice software can report did-not-attend and late cancellations by month; if yours can't, keep a tally for four weeks.

How common are missed appointments?

Common enough to plan for. There's no reliable published rate for private clinics, but NHS figures show how often people miss appointments even when they're free.

NHS England says patients did not attend 16 million GP appointments in 2025, or 1 in 23 (4.3%) of the 376 million arranged. In a Censuswide survey of 2,000 people in England for NHS England, 12% said they'd forgotten an appointment in the past and 11% said they'd arrived too late. These are NHS figures, not private clinic ones, so treat them as a sense of scale rather than a benchmark.

Why do patients miss clinic appointments?

Usually because they forget, because something comes up, or because they couldn't easily cancel. Reminders help with the first; the others need a sensible policy and a way to reach you.

  • They forgot, particularly if they booked weeks ago.
  • They felt better and didn't think to cancel.
  • Work, childcare or illness got in the way.
  • They tried to cancel but couldn't get through, and gave up.
  • They weren't sure what the appointment would cost, or whether their insurer would pay.

The fourth reason is easy to overlook. A patient who rings at 7pm to cancel tomorrow's 9am slot and reaches voicemail may or may not leave a message, and you may not hear it until the slot has passed. Our guide to out-of-hours calls for private clinics covers how to answer those calls.

What should a clinic cancellation policy say?

How much notice you need, what happens if a patient cancels late or doesn't come, any deposit, and how to cancel. Keep it short enough to read on a phone.

  • The notice period, such as 24 or 48 hours.
  • What happens after that: a fee, the loss of a deposit, or nothing the first time.
  • Any deposit: how much, which appointments it applies to and exactly when it stops being refundable.
  • Every way to cancel, such as phone, text or an online link, and that phone cancellations are taken out of hours.
  • That you'll use discretion for genuine emergencies, if you will.

Show it when people book, in the confirmation and in the reminder, not just on a page of your website. A policy the patient never saw is hard to rely on.

Example to adapt

"We ask for 24 hours' notice to cancel or move an appointment, so we can offer the time to another patient. You can call us at any time or use the link in your confirmation. For cancellations with less notice, or if you don't attend, we charge £20 unless we're able to fill the appointment." The £20 is a placeholder: set your own figure against the fairness test below.

Is it fair to charge for a late cancellation or no-show?

It can be, if the charge was clearly agreed in advance and reflects what you actually lose. The Competition and Markets Authority's guidance on writing a fair contract for customers, based on the Consumer Rights Act 2015, says deposits you keep "must take into account what your business will actually lose", and that a cancellation fee must not be excessive and should reflect steps you can take to reduce your losses, such as finding another customer.

  • Keep any fee or deposit modest compared with the appointment price.
  • Don't charge a fee and keep a deposit for the same missed appointment.
  • Waive or refund it if you refill the slot.
  • Apply it consistently, with discretion written into the policy rather than decided case by case.

Other consumer rules can apply depending on how and when an appointment is booked, so if fees or deposits are central to how you run your diary, take advice from a solicitor or your local Trading Standards before you change your terms.

Do appointment reminders reduce no-shows?

They help with one of the main reasons, forgetting, which is why the NHS's own campaign asks patients to turn on reminders and cancel or rearrange appointments they can't make. Send one a day or two before, with the time, the address and how to cancel.

A plain reminder isn't marketing. The ICO says service messages that confirm or remind people about appointments aren't direct marketing. Add an offer, such as money off a massage, and the whole message becomes direct marketing, which brings in the electronic marketing rules. Keep reminders to the appointment, your cancellation terms and how to move it.

How do you refill late cancellations?

Hear about them quickly, and have someone ready to take the slot.

  1. 1Keep a short waiting list of patients who'd happily come in earlier, with the times that suit them.
  2. 2Make cancelling easy at any hour, so you hear about gaps the evening before rather than at the door.
  3. 3When a slot opens, contact the waiting list straight away, in order.
  4. 4Record how many late gaps you refill each month, so you can see whether it's working.

Every cancellation call that goes unanswered is a slot you can't offer anyone. Our guide to what missed calls cost a private clinic puts a number on that, alongside missed calls from new patients.

How Ringzel helps clinics with cancellations

Ringzel is an AI receptionist for clinics. You keep your number and forward the calls you can't take, so a patient ringing to cancel at 8pm reaches someone instead of voicemail.

  • It answers in your clinic's name and tells callers it's automated.
  • It takes cancellation and rebooking requests with the details you ask for, and sends them to you straight after the call, so you can offer the slot to someone else.
  • It can tell callers your cancellation policy in your words, from the information you give it.
  • On Professional and Premium, it can book appointments into the slots you choose.
  • It never gives medical advice and follows your urgent-call instructions as written. If a caller describes an immediate danger to life, it tells them to call 999.
  • After each call you get a recording, a transcript, a summary and a check report. Our privacy policy explains how call data is processed.

Plans are from £99 a month with unlimited calls. You can set up in under 5 minutes, and every plan has a 14-day free trial. Our guide to what an AI receptionist is explains the basics.

Questions about clinic cancellations and no-shows

How do I work out what no-shows cost my clinic?

Multiply no-shows a week by your average appointment price, by the share of those slots you don't refill, and by 52, then take off any deposits or fees you keep.

Can a clinic charge for missed appointments?

It can, if the charge was clearly agreed in advance and is fair. CMA guidance says deposits and cancellation fees should reflect what you actually lose, taking account of steps you can take to reduce the loss, such as refilling the slot.

What notice should a clinic ask for to cancel?

Usually 24 or 48 hours, long enough to offer the slot to someone else. Make it clear at booking and in reminders.

Do I need consent to send appointment reminders?

Not for a plain reminder. The ICO treats appointment reminders as service messages. Add a promotion and the message becomes direct marketing.

What if a patient rings to cancel or rearrange and nobody picks up?

They may simply not attend. Cover that answers cancellation and rearrange calls lets you refill the slot and update the diary instead of discovering a no-show at the appointment time.

Should reminders still go out if the phone is often unanswered?

Yes, reminders cut no-shows, but also make sure patients can get through when they need to change a booking. Reminders without an answered phone still leave late changes stuck in voicemail.

Does Ringzel give medical advice?

No. It takes the details and passes them to you. It never assesses symptoms or suggests treatment.

Do I need a new phone number?

No. You keep your clinic's number and forward the calls you can't take.
