What missed calls cost a private clinic
Guide · ClinicsBy Jon Billington8 min read
A missed call from a new patient costs your clinic what that patient would have spent with you, multiplied by the chance they'd have booked. There's no reliable published figure for private clinics, so the honest way to work it out is with your own prices, typical course of treatment and booking rate. In the worked example below, which uses made-up numbers, four missed new-patient calls a week cost a clinic £14,976 a year.
Missed calls from existing patients cost money too, mostly as cancellations you hear about too late to refill. This guide gives you both sums, shows which calls matter most, explains how to count what you're missing now, and sets out when paying for phone cover makes sense.

How much does a missed call cost a private clinic?
Missed new-patient calls a week, multiplied by the share who would have booked, multiplied by what a new patient is worth to you, multiplied by 52. That's the yearly cost in lost income, before you count unfilled slots.
| Input | Where to find it |
|---|---|
| Missed new-patient calls a week | Your phone system's call log: unanswered calls from numbers that aren't existing patients |
| Share who would have booked | Your own enquiry-to-booking rate, from your diary or practice software |
| What a new patient is worth | Your average spend per new patient over their first course of treatment |

You'll see headline figures for missed calls across all small businesses. A BT Business survey of UK small businesses, published on 1 October 2026, put the average yearly cost of missed calls at £1,597 for businesses with 1 to 9 employees. BT sells its own AI receptionist and describes the estimate as indicative, and it isn't specific to clinics. Your own figures will tell you far more.

| What a new patient is worth | £240 (four appointments at £60) |
| Weeks a year | 52 |

- 14 missed calls × 30% = 1.2 lost new patients a week.
- 21.2 × £240 = £288 in lost income a week.
- 3£288 × 52 = £14,976 a year.
If only one caller in ten would have booked, the same sum still comes to £4,992 a year. Our missed call calculator does the arithmetic with your own figures.
The sum treats every missed caller as lost, when some will ring back. It also leaves out what a happy patient brings later: repeat visits, a review and the friend they send to you. Those pull in opposite directions, so the plain sum is a fair starting point.
What is a new patient worth to a clinic?
Usually more than one appointment. Work it out as your average appointment price multiplied by the number of appointments a typical new patient books in their first course of treatment.
Use your own records rather than a rule of thumb, because it varies so much between clinics. A physiotherapy clinic might see a new patient several times for one problem. An aesthetics clinic might see them once, then again months later. A podiatry clinic might see them regularly for years. Pull the last 50 new patients from your diary and average what they spent in their first three months.
What do missed calls from existing patients cost?
Mostly unfilled slots. An existing patient who can't get through to cancel or move an appointment either doesn't turn up or tells you too late for anyone else to take the time.
As an example only: if two late cancellations a week reach you too late to refill, at £60 an appointment, that's £120 a week or £6,240 a year. Answering those calls doesn't stop people cancelling, but it gives you the chance to offer the slot to someone else. Our guide to clinic cancellations and no-shows covers reminders, waiting lists and fair cancellation policies.
Which clinic calls cost the most to miss?
New patients who want to be seen soon, and existing patients cancelling at short notice. Routine questions from existing patients cost the least, because those callers almost always try again.
| Caller | Why it matters | Will they try again? |
|---|---|---|
| New patient who wants to be seen this week | Often in pain and ringing several clinics | Often not: the first clinic that answers may get them |
| New patient recommended by a friend or another practitioner | Already inclined to choose you | Usually, but it's a poor first impression |
| Existing patient cancelling tomorrow | A slot you could refill if you knew | Sometimes, often too late |
| Existing patient with a routine question | Time, not income | Almost always |

Beyond the money, there's reputation. In BT Business's 2026 survey of 2,000 UK adults, 58% said they'd tell friends and family to avoid a business that didn't pick up. For a clinic that grows by word of mouth, that matters.
How do you find out how many calls your clinic misses?
From your phone system's call log. Two normal weeks show the pattern.
- 1Export two weeks of call history from your phone system or provider's portal.
- 2Filter for unanswered calls and calls that went to voicemail.
- 3Split them by time: during treatment sessions, at lunch, after closing and at weekends.
- 4Mark calls from numbers that aren't existing patients. Those are your likely new patients.
- 5Count how many you called back, and how many of those booked.
- 6Note any cancellations you heard about too late to refill.
In a single-handed clinic, the gaps tend to match your own diary: when you're treating, nobody is answering. Our guide to out-of-hours calls for private clinics covers the evening and weekend share.
When does call cover pay for itself?
When the income it protects is more than it costs. At £99 a month, Ringzel Basic costs £1,188 a year, so with a new patient worth £240 it pays for itself if it saves five new patients a year.
| Option | Cost a year | New patients to break even at £240 each |
|---|---|---|
| An extra full-time receptionist | About £28,300 at the National Living Wage, before cover | About 118 |
| Human answering service | Varies by provider and call volume | Depends on your calls |

| Ringzel Professional | £2,148 paid monthly, £1,790 paid yearly | 9 |

A receptionist does much more than answer the phone, so this isn't like for like. Our guide to what a clinic front desk really costs works that figure out properly. If your log shows very few missed calls from new numbers, you may not need extra cover at all; a clear voicemail and a call-back slot between sessions could be enough.
How do clinic owners without a receptionist cover the phone?
If there is nobody free to pick up, divert busy and unanswered calls to an answering service or AI receptionist on the number patients already dial, so enquiries are captured while you treat.
- Set a no-answer divert, so calls move on after a few rings while you're treating.
- Block a few minutes between sessions to return calls, and keep to it.
- Let people book online for simple appointment types.
- Keep a short waiting list for patients who'd take an earlier slot.
- Have a plan for evenings and weekends, when many patients ring to book or cancel.
- Check the call log monthly, so you see when the pattern changes.

How Ringzel answers calls for clinics
Ringzel is an AI receptionist for clinics. You keep your number and forward the calls you can't take while you're with a patient, at lunch or after hours.
It answers in your clinic's name, tells the caller it's automated and asks your intake questions, such as what they'd like to be seen for, whether they're insured or self-paying and when they're free. It never gives medical advice, follows your urgent-call instructions as written, and isn't an emergency service: if a caller describes an immediate danger to life, it tells them to call 999. After each call you get a recording, a transcript, a summary and a check report, and our privacy policy explains how call data is processed.
Plans are from £99 a month with unlimited calls. Professional adds bookings into the slots you choose, transfers and routing between practitioners. You can set up in under 5 minutes, and every plan has a 14-day free trial.
Questions about missed calls for clinics
What happens if I don't answer? Will patients call another clinic?
Many will. A missed new-patient call is often a booking that lands with a competitor down the road, which is why the cost of a missed ring is usually a full appointment value, not a voicemail.
Is the owner's mobile enough as the clinic line?
It can work if every unanswered ring is diverted to cover when you cannot pick up. A mobile alone, without overflow, still loses calls during treatments and after hours.
How much does a missed call cost a private clinic?
Work it out as missed new-patient calls a week × the share who'd have booked × what a new patient is worth × 52. With made-up inputs of 4 calls, 30% and £240, that's £14,976 a year.
Is there an average missed call cost for clinics?
Not a reliable one. A 2026 BT Business survey put the average for all businesses with 1 to 9 employees at £1,597 a year, but it isn't clinic-specific and BT calls it indicative.
Which missed calls cost a clinic most?
New patients who want to be seen soon, and existing patients cancelling at short notice, whose slot you can't refill if you hear too late.
Does Ringzel give medical advice?
No. It takes the details and passes them to you. It never assesses symptoms or suggests treatment.
Do I need a new number?
No. You keep your clinic's number and forward the calls you can't take.
